Mediafire provide no guidance about how to lodge a DCMA request. So I sent my form email to abuse@mediafire.com. I'll let you know how it goes.
Edited to add: a courteous response and the file removed within a few hours.

You have to look real close, but MediaFire lists a "Report File" link in small font on each individual file page. Still no results, however, after using this feature 3x, as well as e-mailing them directly at the abuse addy twice. Please let us know if you achieve results!
Oops just saw your update. Maybe I'll try emailing them one more time...
Maybe there is an advantage to not using the link (which I didn't see!) ;)
